- Wears gloves when in contact with blood, saliva, and mucous membranes is possible and replaces for each patient or as needed during procedure
- Establishes and maintains the patient record as an accurate and consistent legal document of patient encounters
- Follows-up as necessary with the patient (post-treatment instruction, pain management, self-care)

- Identifies the need and/or performance of and recording of results of other diagnostic procedures and tests

- Collaborates and works effectively with the dentist and other health care providers and community-based oral health programs to provide high-level, patient-centered care
- Transports reusable sharps in a closed, labeled container
- Initiates necessary medical and/or dental consultation or referral in legal media to clarify questions related to the patient's dental and systemic health
- Establishes patient dialogue to obtain demographic, medical, family and dental histories, psychological and socioeconomic information relevant to dental hygiene care; and patient expectations and goals for dental hygiene care
- Establishes patient dialogue to obtain and record the chief complaint of the patient and the history of the present illness
- Obtains, records, updates and organizes accurate and complete medical/dental histories including pertinent psychological and socioeconomic information
- Determines patient needs that can be improved through the delivery of dental hygiene care
- Formulates the dental hygiene diagnosis or diagnoses
- Implements the plan beginning with the mutually agreed upon first prioritized intervention
- Ensures adequate lighting
- Checks procedure trays and waste materials for exposed sharps before handling
- Delivers and/or manages the planned dental hygiene evidence-based treatment and education in sequence and in accordance with accepted Standards of Care

- Provides any necessary post-treatment instruction
- Identifies patient's record including but not limited to medication(s), potential effect(s) on dental and systemic health, and impact on dental treatment
- Utilizes current infection control guidelines for all clinical procedures
- Identifies referrals needed within dentistry and other healthcare disciplines based on dental hygiene diagnosis
- In collaboration with the patient and/or caregiver, prioritize and sequence the interventions allowing for flexibility if necessary and possible
- Follows ethical and legal responsibilities of recordkeeping including guidelines outlined in state regulations and statutes
- Communicates the plan for dental hygiene care to interdisciplinary health team members to determine its congruence with the overall plan for oral health care
- Modifies the plan as necessary and obtains any additional consent
- Counsels and educates the patient and/or caregiver about the treatment rationale, risks, benefits, anticipated outcomes, evidence-based treatment alternatives, and prognosis
- Documents all components of the dental hygiene process of care (assessment, dental hygiene diagnosis, planning, implementation, and evaluation) including the purpose of the patient's visit in the patient's own words
- Removes gloves after patient care and performs hand hygiene immediately
- Identifies the signs of physical, psychological, verbal and substance abuse
- Performs risk assessments
- Records all information and interactions between the patient and the practice (e.g., telephone calls, emergencies, prescriptions) including patient failure to return for treatment or follow through with recommendations
- Follows manufacturer's instructions to safely reprocess dental handpieces and accessories (e.g., low-speed motor, reusable prophylaxis angles)
- Evaluates and integrates emerging trends in health care and determines their value in dental hygiene practice
- Determines priorities and establishes oral health goals with the patient and/or family and/or guardian
- Plans treatments that reflects the impact of behavioral, social and cultural differences in the provision of dental health care
- Applies business and practice management skills to ensure business and practice are reputable and profitable
- Secures the container to prevent spilling contents
- Evaluates and applies contemporary and emerging information including clinical and practice management technology resources
- Confirms the plan for continuing care or maintenance
- Records the final dental hygiene care plan
